Our clients get the continuity of care that stems from having several managers familiar with their needs who can competently step in to provide services. Drawing on the varied backgrounds of the care managers allows us to best serve the medical, psychological and emotional needs of our clients.

Utilizing these resources maximizes the delivery of services and has been invaluable to our clients, especially those experiencing a crisis. At NortheastLink, flexibility is the key to meeting the needs of our clients. We will provide as little or as much service as needed or desired. Each plan is customized and tailored to the needs of the client. The action plan is reviewed and reassessed on a regular basis. It is not unusual to start out with a full component of services and have these services scaled back as the client becomes stronger or more independent. A Care Manager familiar with the clients’ needs is always available.
