Comprehensive Assessments
Objective assessments are completed by a social worker and a nurse to get a full picture of our clients physical, emotional, and cognitive functioning. Includes:
- Review of activities of daily living
- Review of medical records
- Medication management
- Transportation
- Socialization
- Recommendations
- Education
Coordination of Care
Establish a written plan of care based on the assessment. This plan will offer supportive services, alternate living arrangements, coordination of medical services, medication monitoring and escorting to medical appointments. Includes:
- Short term step-by-step plan
- Long range plan for on going concerns
- Transition options
Monitoring
Regularly scheduled visits ensure that the support services meet the goals of the plan and that changing needs are addressed. Includes:
- 24 hour/7 days a week availability
- Scheduling and attending important appointments
- Communicating with family to avert a crisis
- Checking in by phone
Advocacy
Linking the client with community resources for guidance in financial, legal and insurance matters. Assisting in maximizing and extending resources and navigating the complex healthcare system while utilizing finances effectively. Includes:
- Assistance with transitions to Assisted Living or Nursing Home
- Applying for community programs
- Dealing with insurance companies
- Caregiver education and support groups
- Emergency contact when caregiver is unavailable
