NortheastLink consists of a team of seasoned geriatric professionals trained in either social work or nursing, each of whom brings specific expertise within either the hospital, home care, residential or adult day health setting

Lisa Hooper

Director of Community Care Management for NortheastLink
Lisa Hooper

Lisa holds a Bachelor’s Degree in Business Management from Lesley University and has 20 years of healthcare and business experience. Lisa served for the last ten years as Director of Operations for a large home health agency. Lisa has worked with a variety of clients and their families to foster independence, offer timely support, and dependable care for those with dementia and physical illnesses.

Darcey Adams, MSW, LICSW, C-ASWCM

Darcey Adams

Darcey earned a Bachelor’s Degree in psychology from Keene State College and a Master’s Degree in Social Work from Boston University. Darcey has over 15 years of medical social work experience in a variety of leadership positions. Darcey presently has the oversight responsibility for the dementia-specific Spectrum Adult Day Health Program, and NortheastLink, a geriatric care management program. Darcey is president of the Massachusetts Adult Day Services Association and a member of the New England Chapter of Geriatric Care Managers. Darcey is also a Certified Advanced Social Work Case Manager (C-ASWCM) under the National Association of Social Workers (NASW).

Jackie Buckley, RN, BSN

Jackie is a graduate of St. Anselm’s College and has over 30 years of nursing experience including VNA, Adult Day Health, 10 years in Elder Case Management, and Assisted Living (both traditional and Alzheimer’s/dementia-specific programs). She is an Alzheimer’s disease educator and support group facilitator. Jackie completes clinical assessments and coordinates the care needs of clients by collaborating with physicians and other caregivers.

Brianna Martin, MSW, LCSW

Brianna holds a Master’s of Social Work degree from Boston College and a Bachelor’s of Arts degree from Endicott College. In addition to completing the Practicum Partnership Program at the Boston College School of Social Work, she is also certified as a trainer by the Alzheimer’s Association MA/NH Chapter, an organization she has also been involved with as a volunteer. Her previous professional experience includes roles as a client care manager and a placement coordinator/caregiver liaison for two large home care agencies, positions in which she managed services for clients and their loved ones, conducted in-home assessments and assisted with care coordination and service referral. In her role at NortheastLink, Brianna provides a range of comprehensive care management services to clients and their family members using a multi-disciplinary team approach.

Brooke Parker, MSW, LCSW

Now a Care Manager for NortheastLink, Brooke began as an Intern/Assistant Care Manager. In addition, she previously served as an Intern at Health & Education Services’ Center for Family Development, an Instructional Aide for SEEM Collaborative Middle School in Stoneham, and a Volunteer Events Coordinator for the Susan Bailis Assisted Living Community in Boston. Brooke holds a Bachelor of Arts degree in Communications from Boston University and a Master’s degree in Social Work from Salem State University. In her role, Brooke provides a variety of care management services to NortheastLink clients such as advocacy and supportive care. She seeks to promote a client’s independence and sense of well-being.

Georgia Stewart

Before moving to the United States from Australia, Georgia was trained as a psychiatric nurse. She has over 20 years of experience in the field of mental illness with recent training in Alzheimer’s disease. Her knowledge of mental health issues has prepared her to understand the subtle changes in mood and behavior in clients and to communicate these changes to the other members of the team.
